Make a Formal Complaint Against a Police Employee
If you have questions or concerns about your interaction with a police officer, contact (405) 231-2121 and ask to speak to a supervisor. A supervisor will call or meet you in person to address your concerns.
You can file a formal complaint with the Oklahoma City Police Department by filling out a formal complaint form. Formal complaint forms can be picked up at any police station or downloaded at the following link:
This form should only be used to make a complaint against an OKCPD officer. View information on how to report a crime.
Once your form is complete, it can be dropped off at any police station, or a supervisor can be called to retrieve the form. You can also mail the completed form to:
