Records Request Form

The Records Unit is a vital part of the Oklahoma City Police Department. Records management is critical to the operation of the entire department. The Oklahoma City Police Department Records Unit began operations on Aug. 23, 1910. The Records Unit operates 24 hours a day, seven days a week and exists as a safekeeping repository for case history documents from all divisions of the Oklahoma City Police Department. Records staff audit and review crime reports submitted by officers and through the public self-reporting crime portal and maintain crime statistics for the department and report these statistics to state and federal agencies. The Records Unit operates a customer service section to provide access to crime reports via an online request portal and a public counter located at 700 Colcord Dr., open Monday-Friday, 7:30 a.m. to 7 p.m.

Theft/Loss of Property Form (PD-16)(PDF, 202KB) 

PD-16 explanation:

A PD-16 form should be used to inventory any items discovered stolen after the original burglary or theft report has been filed. Please note the case number assigned at the time of the initial report and include it on the completed PD-16 form. The form may be submitted by email, mail, or in person at police headquarters or any police briefing station.

Permits & ID Office

The Permits and ID Unit, located at 616 Colcord Drive, operates Monday through Friday from 8 a.m. to 4 p.m., excluding weekends and City-approved holidays. The unit issues and renews alarm permits and enforces related ordinances. For more alarm permit information, go to okc.gov/alarms.

The Permits and ID Unit also issues vehicle for hire (VFH) business licenses, inspects vehicles for hire (e.g., taxis, limos, airport vans), issues VFH driver permits, investigates VFH complaints, and enforces related ordinances. For more VFH information, go to okc.gov/vfh.
