Preexisting Conditions

A pre-existing condition exclusion will apply to expenses involving the replacement of teeth that were missing prior to the effective date of the dental contract. This exclusion will not apply to:

  • Any participant who becomes effective on the dental contract date who was covered under a previous group dental care contract by the Employer.
  • Any participant who has been continuously covered for 24 months under a group dental care contract with BlueCross BlueShield of Oklahoma which included prosthetic benefits.

Limitations

When the course of treatment will be in excess of $300, a predetermination request should be submitted to BlueCross BlueShield of Oklahoma in advance of treatment.

  • Low Plan Option
    When seeing a Non-Contracting Dentist, your out-of-pocket cost will be greater because Non-Contracting Dentists have not entered into a contract with BlueCross BlueShield of Oklahoma to accept any Allowable Amount determination as payment in full for Eligible Dental Expenses. Claims will be reimbursed at the Maximum Allowable Charge (MAC). You are required to file claim forms. You are balance billed for costs exceeding the BlueCross BlueShield of Oklahoma Allowable Amount.
  • High Plan Option
    When seeing a Non-Contracting Dentist, your out-of-pocket cost will be greater because Non-Contracting Dentists have not entered into a contract with BlueCross BlueShield of Oklahoma to accept any Allowable Amount determination as payment in full for Eligible Dental Expenses. Claims will be reimbursed at the 90th percentile. You are required to file claim forms. You are balance billed for costs exceeding the BlueCross BlueShield of Oklahoma Allowable Amount.

Health Flexible Spending Account (FSA)

A Health FSA allows you to allocate money on a pre-tax basis to reimburse yourself for qualified medical expenses for you and your family. Qualified expenses include anything from co-payments, medical deductibles, prescriptions, and much more.

Minimum Annual Deposit: $120
Maximum Annual Deposit: $2,650 (for 2019)

Health FSA Debit Card

American Fidelity will provide a Debit Card to all employees who elect to participate in a Health FSA. The debit card gives immediate, convenient access to Health FSA Funds at the point of sale for prescriptions, copays, and other common qualified medical expenses. This card can only be used for the Health FSA and is not available for the Dependent Care account.

Dependent Care Flexible Spending Account (FSA)

A Dependent Care Account allows you to allocate money on a pre-tax basis to reimburse yourself for dependent care services such as after school care and dependent daycare centers.

Minimum Annual Deposit: $240
Maximum Annual Deposit: $5,000

Partial list of Eligible Dependent Care Expenses:

  • After-school care or extended day programs
  • Nanny expenses
  • Baby-sitter inside or outside participant's household
  • Custodial or elder care expenses if the qualifying individual still spends at least 8 hours each day in the employee's household
  • Dependent care center* expenses/pre-kindergarten/nursery school expense if primary purpose is to care for the child so the parent can work
  • Expenses paid to a non-dependent relative of participant
  • Summer day camp if the primary purpose of the expense is custodial in nature and not educational

*A dependent Care Center is a place that provides care for more than six persons (other than persons who live there) and receives a fee, payment or grant for providing services for any of those persons, regardless of whether the center is run for profit.

Taxpayer Information

Regardless of whether you participate in the dependent care plan under Section 125 or claim the credit on your income tax, you must provide the IRS with the name, address, and taxpayer identification number (TIN) or Social Security number of your dependent care provider(s) by completing either Schedule 2 of form 1040A or Form 2441 and attaching it to your annual income tax return. Be sure that you follow the current instructions given by the IRS for preparing your annual income tax return. Failure to provide this information to the IRS could result in a loss of the pre-tax exemption for your dependent care expenses.

AFmobile® allows FSA participants to submit Health FSA, Dependent Day Care FSA  claims while on the go! Our mobile app allows customers to:

  • Submit FSA claims by taking pictures of receipts,
  • Sign-up for Flex Direct Deposit, and
  • Create their Online Service Center accounts.

The City of Oklahoma City offers employees a choice in major medical plans: the Group Indemnity Health Plan [PPO and EPO (new for 2024) Plans administered by BlueCross and BlueShield of Oklahoma]. Only you can decide the type of major medical plan that is right for you and your family.

Group Indemnity Plan

The Group Indemnity Health Plan offers a broad network of doctors, allowing you the ability to select almost any doctor or hospital. By selecting a network doctor, lower coinsurance and deductibles are available. However, non-network care is still partially covered on the PPO plan. A prescription drug plan is offered with the Group Indemnity Plan. Prescription drugs must be included on the plan formulary in order to be covered.

The EPO plan offers the same national network and basic drug list as the PPO, but with no deductible or coinsurance to help minimize out-of-pocket costs to you. However, the EPO does not offer a separate out-of-network benefit like the PPO plan.

PPO Deductibles

A deductible is the portion of eligible medical expenses that the participant must pay before the plan will make any benefit payments. For example, with a $250 deductible, if the eligible amount of your first medical expense is $150, you will be responsible for the entire amount, and $150 will be applied towards your deductible. If then eligible amount of your second medical expense is $250, you will be responsible for the first $100, which will be applied toward your deductible. The remaining $150 would then be eligible for payment by the plan.

PPO Coinsurance

Coinsurance is a percentage that the patient is required to pay on all eligible medical expenses, after the deductible has been met. For example, if your deductible has been met and you have an eligible medical expense of $200, your coinsurance (10%) would be $20, and the plan's responsibility (90%) would be $180. The patient's coinsurance on out-of-network expenses is 30%.

PPO Out-of-Pocket Maximum

An annual out-of-pocket maximum is the maximum amount of coinsurance and copayments you pay for eligible medical expenses in any single calendar year. Once you have paid the out-of-pocket maximum, the plan pays 100% of eligible expenses.

Blue Access Mobile makes it easy for you to access your information while on the go. You can view coverage details, health and wellness information, check claim status and access ID card information. You can also sign up to get text or email alerts and tips for:

  • Claims Information — Get alerts when a claim status changes.
  • Exercise and Fitness — Get weekly tips to help improve your workouts.
  • Prescription Drugs — Sign up for alerts to check blood sugar, take a medication or a multivitamin.
  • Diabetes Diet — Get weekly diet management tips to help stabilize insulin levels.
  • Heart Healthy Diet and Basic Care Management — Get weekly messages with heart healthy diet management and basic care tips.

Pharmacy Benefits

Coverage of prescription medications on the Group Indemnity Plan is provided by Prime Therapeutics.

Prime Therapeutics also provides a mail order prescription drug program.

Group Term Life

The City provides $20,000 in basic life insurance and an additional $5,000 in Accidental Death and Dismemberment Insurance at no cost to the employee.

Voluntary Supplemental Life

If you purchase the optional supplemental life coverage, you may also purchase voluntary supplemental life coverage at 1/2x, 1x, 2x, or 3x your annual base salary. Life insurance will be rounded to the nearest $1,000, if not already a multiple thereof. Note: Maximum coverage for basic, optional, and voluntary life insurance combined is $500,000.

Dependent Life Insurance Coverage

You may purchase Voluntary Dependent Life insurance for a qualified dependent spouse and/or child(ren) as follows:

Spouse: Spouse coverage may be elected for one of the following amounts: $10,000, $20,000, $40,000, $60,000, $80,000, or $100,000.

Child(ren): Age live birth to 6 months - $100. 6 months to 23 years - Child coverage may be elected for one of the following amounts: $2,500, $5,000, $7,500, $10,000.

Coverage is available for eligible dependents of active, full time employees; required documentation must be on file with the Employee Benefits Division of the Personnel Department.

Accidental Death and Dismemberment Coverage

The City provides $5,000 of AD&D coverage for each full-time employee. Employees may purchase an additional $5,000 of AD&D coverage. AD&D coverage provides benefits for an accidental death, and for an accidental dismemberment, as defined in the schedule of benefits.

Age Reduction

Coverage amounts will reduce according to the following schedule:
Age 65 - 65% of the original amount
Age 70 - 40% of the original amount
Age 75 - 25% of the original amount
Coverage may not be increased after a reduction.

Portability and Conversion

At the time an employee leaves the city or otherwise becomes ineligible for City benefits, that employee may be eligible for portability or conversion of their life insurance.

Conversion

Converts life insurance into a whole life policy which can generate cash value after being in force for a number of years.

Conversion locks you into a specific rate, based on your age at the time you convert. The younger you are at the time of conversion, the lower your rate.

Conversion amounts do not contain age reductions for the insurance coverage you convert.

You may convert all or part of your basic (employer paid) life insurance.

You may convert all or part of your voluntary (employee paid) life insurance.

Spouse coverage can be converted.

Portability

There is no cash value associated with a ported policy of coverage. The policy continues to be a term life policy.

Portability rates change (rise) as you age, in five year increments. For example, age 45-49, age 50-54, etc.

Portability amounts do not contain age reductions for insurance coverage you port.

You may NOT port your basic coverage.

You may port all or part of your voluntary life insurance.

Spouse coverage can be ported.

Individual Term Life

American Fidelity offers an Individual Term Life Insurance policy to help with your financial needs for your short-term and long-term goals.

How the Plan Works

Individual Term Life Insurance is a death benefit protection with no cash accumulation feature. The policy is initially written for a 10, 20 or 30-year term period but may be renewed at the insured’s option for the same level renewal period depending upon the term chosen.

The last level renewal period is no later than age 70 for the 10-year term policy and age 60 for the 20-year term policy. Thereafter, premiums are renewable annually up to age 90. The 30-year term policy is renewable annually after the initial 30-year term period up to age 90. Renewal rates will be based on the insured’s age at the time of renewal.

2025 Open Enrollment Information

Mark your calendar: Onsite open enrollment will be held at the OKC Convention Center, Oct. 28 – Nov. 1. Self-Service will be open from Oct. 22 - Nov. 4.

You should receive your 2025 Benefit Guide beginning the week of October 14.

Please review the changes below for the 457 Deferred Compensation Plan for 2025.

To enroll on-site, you need to bring your registered MFA with you. This could be a USB security key (FIDO) or an MFA registered mobile phone for text message or the Microsoft Authenticator app. Employees who have set their MFA to ring at their desk phone number will not be able to complete on-site enrollment unless they have another registered MFA type. If you have never used a City computer, your IT Contact or supervisor will provide instructions on how to register your account.

All employees are encouraged to visit on-site enrollment to meet with American Fidelity and other vendors.

Additional benefits information will be communicated to departments and employees before open enrollment.

FAQs:

  • What is MFA?
  • MFA is multi-factor authentication. It is a security process that requires users to provide two or more separate passwords, codes, or PIN before granting access to a computer system or account.
  • What is a FIDO key?
  • FIDO is Fast ID Online. A FIDO security key is a small device that plugs into a USB port on your computer. It is used to confirm your identity when logging in to the City's network. FIDO keys are one of the available MFA options. 
  • Who needs a FIDO key?
  • All employees need to have a registered MFA so they can have access to computer-based employee resources such as paycheck details, benefits, and job applications.  FIDO keys are one of the available MFA options. They are the required MFA type for employees who use shared computers such as at a shared shift desk, lobby kiosks, training rooms, and conference rooms.  

ADDITIONAL OPEN ENROLLMENT NOTES

1) ADDING SPOUSE/DEPENDENTS - If you need to add a dependent and/or spouse, we will require the following documents:

  • Child- Copy of State Issued Birth Certificate and Social Security number (Copy of card is preferred)
  • Spouse-Copy of Marriage Certificate and Social Security number (Copy of card is preferred). Please also provide date of birth.

Documents can be uploaded into One OKC Cloud as part of self service.

2) GROUP LIFE INSURANCE -  Any increase to voluntary or spouse life requires an Evidence of Insurability (EOI) to be completed and mailed to BCBS (Dearborn) Life for approval. Once Employee Benefits receives the approval, we will update your elections. Oracle Cloud will not allow an increase in self-service that requires a EOI. Once benefits receives approval of EOI from BCBS Life, benefits will process the increase at that time.

3) GROUP LIFE BENEFICIARY - Beneficiaries should be updated in One OKC Cloud. As part of conversion, prior beneficiary information was not included but was archived.

4) Flex Spending Accounts - Flex spending does NOT renew each year. You are required to re-elect Flex Spending each year. If you need to have a Flex Spending Account, you will need to enroll in One OKC  Cloud during Open Enrollment.

**NEW**Deferred Compensation Plan Administrator

The City of Oklahoma City is moving to a single administrator, Voya Financial. This will lower costs for you, provide greater fee transparency, improve services and simplify account management.

What you need to know -

MissionSquare and Nationwide are the current recordkeepers for the City of Oklahoma City’s deferred compensation plans. Any account you currently have with either MissionSquare or Nationwide will be transferred automatically to Voya® in December 2024.

The transition to Voya will begin at 3 p.m. CT on December 10, 2024, and is expected to be completed the week of December 23, 2024.  During this time, you will not be able to make any changes to your account.

What action do I need to take -

Your account details, if applicable, including your investments, contribution rate elections, and beneficiaries, will transfer to Voya automatically. You are not required to take any action.

Where can I learn more information -

Voya will be at the Onsite enrollment October 28 to November 1 to discuss plan changes and investment options. 

There is also a short group meeting at 9a, 10a, 11a, 1p, 2p and 3p each day during Onsite enrollment. The meeting should take about 20 minutes.  Stop by and learn more about the 457 plan changes and about Voya Financial.

The City provides Tuition Reimbursement for full-time employees for most pay plans, which provides reimbursement for tuition and required fees, up to $1,250 per semester.
