Key to Home expands CARE Team with support from OCCF

The Key to Home Partnership launched the Coordinated Adaptive Response and Engagement (CARE) Team earlier this year to create a new pathway to housing for people with complex mental health challenges.
The CARE Team takes a multidisciplinary approach to homelessness, with staff experienced in street outreach, first response, mental health and clinical care.
The new program is being funded through Oklahoma City Community Foundation's Bridge OKC Fund, which was created to help bridge critical gaps in nonprofit support and meet the community's most pressing and widespread needs across central Oklahoma.
“The CARE Team project represents exactly the kind of collaborative, community-informed solution that meaningful change requires,” said Trisha Finnegan, President and CEO of the Oklahoma City Community Foundation. “Investing in a team that can connect with our most vulnerable neighbors is how we support lasting stability. We're proud to help the Key to Home Partnership grow this work.”
Support from the Oklahoma City Community Foundation will help expand the CARE Team's impact and will secure rental assistance for clients while they work a plan with case managers toward stabilization.
“We’re excited to be able to provide a higher level of care for our neighbors that have more complex needs,” said Key to Home Partnership Manager Jamie Caves. “Our service providers work incredibly hard to provide care and support for every neighbor in need, but historically, our system just hasn't had the resources available to meet the needs of this population. Thanks to the support of the Oklahoma City Community Foundation, we are able to dedicate the resources to finally make meaningful progress for this population.”

Oklahoma City Community Foundation's Bridge Fund is designed to support data-driven projects that can make a real difference for individuals and the broader community. To support this important work, OCCF, through the support of its Trustees and generous donors, awarded $794,750 to three organizations: Key to Home Partnership, Homeless Alliance and Mental Health Association Oklahoma. One additional grant will be made at a future date, bringing the total funds invested to more than $1 million.
